- Duly Accomplished, signed, and notarized Personal History statement of proprietor/each partner/each incorporator/director
- NBI clearance of proprietor/each incorporator/ director
- Duly accomplished, signed and notarized Sworn Statement of Assets and Liabilities of proprietor/each partner/each incorporator/director
- Fill out the authorization form to allow PAGCOR’s technical inspection team to conduct a site inspection
