  9. Duly Accomplished, signed, and notarized Personal History statement of proprietor/each partner/each incorporator/director
  10. NBI clearance of proprietor/each incorporator/ director
  11. Duly accomplished, signed and notarized Sworn Statement of Assets and Liabilities of proprietor/each partner/each incorporator/director
  12. Fill out the authorization form to allow PAGCOR’s technical inspection team to conduct a site inspection
